This is a brutal and disturbing cover art. It features Dead's (Per Yngve Ohlin) corpse after he killed himself in 1991. Euronymous (Øystein Aarseth) took this picture after he found out. This cover art is featured on the 1995 live album ''Dawn of the Black Hearts'' by Mayhem. Really twisted.
