Maniac of Mayhem on Deathcrush sounded really savage. Wolf's Lair Abyss performance was also remarkable in this sense. On Chimera he sounded way more controlled and calculated. As for Sean Killian, well, the dude does sound kinda psychotic on the debut album. The next records showcase some of his other qualities as a singer, but that touch of craziness is still there.

Paul motherfucking Baloff. This guy didn't give a fuck that the riffs were maybe a tad heavier than what Metallica and Megadeth had done before. As feral as Mayhem were on Deathcrush, it's another thing when the vocalist is that kinda feral but everyone else isn't, and yet it fits in this fucking bizarre way - I can never seem to put my finger on it.

To a smaller extent, I really really want to say Dave Sanchez during those moments (especially in the past couple of Havok albums) where he goes from those generic thrash snarls into some fucking Paul Baloff meets Dave Hunt style shit.

On that note, yyyyyyyyyyyeah, the thing with Dave Hunt is that on the early Anaal Nathrakh stuff, he definitely sounded unhinged; but as time went on (basically, as we went into the 2010s, and possibly during Constellation back in 2009), forget a mere distortion pedal - guy was relying on all kinds of vocal effects, well past the point where even Ozzy Osbourne would think that's too much. Love the band for the most part, but there's no denying that.

Arioch/Mortuus’s vocals in Funeral Mist. He is all over the place and absolutely bombards you like a demon possessed. Seconding the Sylvain Houde, Landfermann, Lori Bravo and Lord Worm suggestions.

Neil/Lord Imperial from Krieg is an underrated vocalist, maybe wouldn’t say he sounds unhinged but he’s got a very strong sound.

I haven't heard most of what he's performed on, but I always though Martin van Drunen sounded pretty unhinged.

I don't always find Luc Lemay's vocals to sound completely unhinged, but the high-range shrieks on this particular song always sound utterly deranged to me.

YES! Flames of Hell- one of the most obscure albums out there (though maybe not any more thanks to the magic of modern Youtube) and whether or not Burzum/Varg was directly influenced by it or not- I would assume not, given how obscure it was back then- the similarity of the music and vocals to early Burzum material is very noticeable.

The two aren't mutually exclusive. Regardless of how they're made, I would definitely describe the vocals as sounding "unhinged."

I think Sylvian Houde from early kataklysm is probably the most unhinged, although i'd say that the singers of Revenge and Anaal Nathrahk could fit the bill as well. The best unhinged vocalist though has got to be Lord Worm, I mean "Open Face Surgery" and "Slit Your Guts" vocals are all over the place but yet it sounds so perfect for the music.

Lori bravo on Carrion for worm is absolutely just fucking insane it's a beautiful thing really.

MVD on Consuming Impulse just gives me chills thinking about it, holy fuck is that album just completely fucking brilliant.

My man Dani Filth on Dusk... And Her Embrace just batshit fucking crazy Vocal performance on that one

Craig Pillard on OTG is just so intimidating and evil that grunt alone would stop any macho assholes in their fucking tracks (PS. Kinda sucks ass about that whole disma thing)

And the bands Human Remains and Ripping Corpse are just ridiculously unhinged to begin with

Yea i'm surprised I forgot about Intestinal Disgorge. Not metal, not even a good band tbh, but I do enjoy the song prostitute by them. They definitely have unhinged vocals.

Rainer from Bethlehem, Onielar from Bethlehem & Darkened Nocturn Slaughtercult. Alan Dubin of Khanate sounds deranged. I also think Charlie Fell of Lord Mantis sounds disgusting in a great way. Dax Riggs I think often can sound very unhinged especially accounting for lyrical content. Around V Halmstad Niklas was great as was Malefic of Xasthur. But I think for DSBM vocals the line between unhinged and cringy is close. The singer of Advent Sorrow is among one of the more disturbed sounding vocalists of recent times.

They might be a meme-ey band but Dickie Allen from Infant Annihilator has one of those unique (pretty "unhinged" IMO) tones you recognize anywhere. Versatile as hell too. I'd love to see him in a "serious" depressive black metal band or something. They seem to get more "metal" with each album so maybe one day.

Depending on what you're thinking, the vocalist of Hexecutor definitely sounds completely unhinged. That shit definitely can't be good for your voice.

The vocals on the first Slipknot album were pretty uneasy in a lot of parts, mainly the one that comes to mind.
