Fluids are very similar to Mortician, but I feel like Fluids is an uglier band. They have a tendency to use audio from real gore videos like cartel torture videos and shit, that shit just leaves me wanting to take a long cold shower.

Seriously. I really like Fluids a lot but their samples are actually hard for me to listen to and sometimes I skip through them.

And that's coming from someone who's a HUGE fan of true crime documentaries and true crime books, reading about and watching stuff on serial killers and watching horror movies, but when you get into REAL VIDEO, PHOTOS AND REAL AUDIO OF TORTURE AND MURDER, I WANT NOTHING TO DO WITH IT!!

The whole thing is that true crime is fascinating, but it doesn't feel real to me and so I can take it. It's like even though I know the stuff really happened, it won't compute in my brain as really real, but the few times I have accidentally seen actual death videos I have never liked it at all, though I think often I have still been desensitized and not reacted as strongly to them because it's still hard to believe they are real.

As soon as shit hits home as being real then all my interest is lost.

Fluids are like Mortician but more "real" and that makes them more brutal in my eyes, but both bands are great.

I think the difference is that true crime is curated content. There might be a few pictures or audio snippets here and there, but nothing as graphic as straight up looking at/listening to someone getting their face sanded off with a power tool.
Musically Fluids is right up my alley, but like you said the samples makes it hard to listen to.

I don't really enjoy this, but this is by far the most brutal piece of art I've ever heard:

Maybe Pharmacist is what you're looking for? Japanese goregrind, very similar to Pathologist. Not quite as fuzzy and slimy as a lot of stuff in this thread.

Also try the Autopsy album Mental Funeral if you're looking for a little moldy OSDM.

Encoffination can be so joyfully ugly and heavy.

Spoiler: show

So since you're a bit vague (on purpose, I assume!), here are some of the filthiest, darkest bands I know, but they may not be in the like precise styles you're looking for

GOATROT
Úzkost ex-Slaves BC)
Blasting Blood
Desmodus Rotundus
Lucifer Impaled
Via Crucis (USA)
Satan Destroyer
Frozen Burial (country unknown, NOT the usa band)
Brainkrusher
Reverorum ib Malacht
Slammed into Oblivion

If you want some truly emotionally ugly music that evokes homocidal / suicidal themes then you can't go wrong with sludge bands like Indian or Khanate.

Bestial Putrefaction. They combine war metal with goregrind. Some of the nastiest shit I've ever heard.

If you want dissonance and just strange music then i would go for Portal or Blut aus nord.
If you want heavy music then i would go for Mortician.

For me personally the album Exit by Rotten Sound.

I just listened to some Deiphago earlier today. I gotta say, that is some dense, chaotic, hateful music.

Funnily, Blood Feast were marketed as "ugliest band in the world" in mid/end-80es when they had just released their "Kill for Pleasure" debut, an incredibly ugly brother of the Exodus' debut. At least in Germany this was in the promo papers and quoted on basically all reviews.

Seconding Deiphago. Their last album is a complete mindfuck of brutality and sharpness.

Just start listening to goregrind, your friends will respect you and your neighbours will love you. Torsofuck and Rectal Smegma are a good place to start.
