The intro to Twin Peaks cannot be beaten.

Second would be Trailer Park Boys, and then The Simpsons simply because I've loved that show for 32 years.

Archer comes to my mind. The theme song is excellent (love that "descending" piano) and the animations are great. The first few times they almost fool you into thinking this will be a fairly serious show and not 20 minutes of pain and suffering a time which you will keep coming back to.

Barney fucking Miller, if only because anytime I get handed a bass guitar, I have to play that opening bass riff. Of all the great basslines that exist, it's still the one I think of first and instinctively play. It's a problem I have.

The intro to the original version of the movie The Shining movie is creepy as hell.

I like the intro to the movie Speed, it pumps you up. Also, most Danny Elfman movie scores, intro's are awesome.

Twin Peaks and Zombieland are my favorites out of the ones that were mentioned... But my absolutely favorite opening title sequence of all time (for a movie) is Seven. I still remember how striking it was when I first saw it at the cinema. It perfectly hints at the movie you're gonna see, and the choice of music was also genius and perfectly fitting.

Intro-wise I think I'd pick Buffy, Friends, and House - they put in a lot of effort with the whole package, updated them as a the series' progressed, and they still stick in yer mind now. It's always a bit sad when modern programmes don't bother with them.

One that sticks in my head (that I never watch myself) is the Match of the Day theme on the BBC - look it up, great little earworm. Reminds me of Sunday afternoons with my grandmother - my uncles (who must have only been in their twenties with hindsight, weirdly large families) lying around watching the football.
